Great News - Applications are being developed for “other control devices” as well as “other assistance considerations” these will be posted on the website when available; for now please use the current application for requests for assistance through S.U.G.A.R.

At this time applicant must be a resident of Canada

All applicants must sign a Release of Liability and a Declaration of trust.

The review process involves 4 stages, during which the applicant may be contacted as they pass each stage.  The applicant will definitely be contacted at phase one and at the final review phase.

The application original and supporting documents must be sent via post office mail or courier (however we prefer applicants to keep their costs down) to the S.U.G.A.R. address located on each page.

If you do not hear from S.U.G.A.R. within two weeks after mailing your application, please email or phone to find out if it arrived and to trace it down or send out a new one.

Please allow sufficient time for review of the application.  

Applicants like to tell us more about themselves to complete a broader picture, so when completing the application feel free to add cover letters or comments to back up your information.  S.U.G.A.R. is here to help.

S.U.G.A.R. applications: 

Please include your name and mailing address for our records.

For an application package to be post office mailed to you: email info@sugarcharity.org , or call 905-632-0932—leave a clear message saying and spelling your last name, a phone number where you can be contacted at.  S.U.G.A.R. will return your call and get the information necessary to mail an application package directly to you.          

Please follow up if you do not receive the application, as we cannot possibly return calls to everyone who requests the application.

Privacy:

Each application must be completed in full as  S.U.G.A.R. will not consider any applications that have not been completed when submitted.  With that in mind, every single application submitted to S.U.G.A.R  is protected for your privacy.  No names or other personal information is shared, upon review of the file all personal information other than the applicant name and address is sealed.
